V patogenezi (v časné fázi) se výrazně uplatňuje endotelová dysfunkce. Roli mají zánětové děje, růstové faktory a jiné cytokiny. Pro vývoj komplikací je důležitá stabilita aterosklerotického plátu, jehož ruptura může vést k vzniku nasedající trombózy a provalení ateromových hmot. Léčba a. je dosud zaměřena především proti komplikacím či naopak rizikovým faktorům, zejm. ovlivnění lipidového metabolismu. U žen se výskyt a. zvyšuje po menopauze.
